Full Job Description

NHS Midlands and Lancashire (ML) is seeking an enthusiastic, highly motivated individual with excellent communication, interpersonal and organisational skills to work within the PHC Scheduling Team. To arrange/schedule patient care assessments and reviews for case management clinical colleagues - contacting individuals, family members/representatives and other relevant professionals as required., This is a busy team that supports the overall Personalised Healthcare Service to ensure that patients/representatives have a positive and seamless experience.

The main duties of the job are arranging patient assessments and booking these into our clinician calendars and liaising with patient's next of kin and social services.

The post holder will be required to work under their own initiative as well as working as part of the wider administration team. Excellent interpersonal skills and communication skills are essential plus an ability to deal with highly sensitive and confidential information.

Please note that this role is hybrid working from home but the post holder will be required to attend the office for meetings, Appraisals and essential onsite working at Cardinal Square, Derby.

This is a busy and demanding role and the ability to prioritise whilst multi-tasking is essential.

The Personalised Healthcare (PHC) service is commissioned by the Integrated Care Board (ICB) to provide PHC services for the Derby and Derbyshire Integrated Care Board.
