Full Job Description

The Crisis Response and Home Treatment Team is a key part of the Urgent Care Pathway meeting the demands of the Ten-Year Plan to provide Crisis Response and Home Treatment Services by 2021. Clinicians employed into the team are part of an exciting and developing service aimed at supporting our most vulnerable patients and their families in the least restrictive setting possible by providing a variety of highly intensive interventions at home., We require RMNs/clinicians who can bring their own professional expertise to a fast-paced team, and who are confident in managing risk in the least restrictive way. By its nature, crisis work requires professionals who have a broad range of skills including being skilled mental health assessors with a proven ability to signpost service users towards the most effective treatment pathways.

Crisis can manifest with a range of needs and whether you have a background in mental health nursing, social care or occupational therapy we want you to use your skills as part of a team to develop individualised care packages for our service users and their families.

Service Users access a crisis response in a variety of ways, ranging from referrals from their GP and attendance at an emergency department, to when they need care in the community and are seen by the police and our street triage teams. The aim of the Urgent Care Pathway is to bring together all those access points under one team, including:

  • Psychiatric Liaison

  • Overnight Response Team

  • Assessment Teams

  • Street Triage and Health Based Places of Safety
