Full Job Description

We are currently seeking a dynamic and experienced Area Care Assessor to join our team. The Area Care Assessor will be responsible for overseeing the care services within a designated geographical area, ensuring the highest levels of care are maintained, fostering a culture of excellence, driving operation efficiency across multiple service areas and operational objectives are achieved., As an Area Care Assessor, you will be a vital link between our clients and our care team. You will conduct comprehensive assessments to determine the care needs of new and existing clients, ensuring they receive the best possible support tailored to their individual requirements. This role requires excellent communication skills, a keen eye for detail, and a genuine passion for helping others.,

  • Conduct thorough assessments of clients' care needs in their homes within a 40-mile radius of Sudbury.

  • Develop and review personalized care plans in collaboration with clients, their families, and our care team.

  • Regularly monitor and evaluate the effectiveness of care plans, making adjustments as needed.

  • Maintain accurate and detailed records of assessments and care plans.

  • Provide support and guidance to clients and their families, ensuring they understand and are comfortable with the care provided.

  • Work closely with care coordinators and care workers to ensure seamless delivery of services.

  • Stay informed about best practices and regulatory requirements in domiciliary care.

    Previous experience in a similar role within the healthcare or domiciliary care sector.

  • A relevant qualification in health and social care (e.g., NVQ Level 3 or above).

  • Strong assessment and care planning skills.

  • Excellent communication and interpersonal skills.

  • Ability to effectively manage multiple priorities, work under pressure, and adapt to changing circumstances and business needs.

  • Ability to work independently and manage your own schedule.

  • Full UK driving license and access to a reliable vehicle.
